Cardiology Billing 101: CPT Codes, Modifiers, and Pre-Auth Strategies

Cardiology is among the most complex specialties to bill, thanks to the range of diagnostic tests, imaging studies, and interventional procedures. This guide covers the essential CPT codes, modifiers, and pre-authorization habits that keep cardiology claims clean.

Essential cardiology CPT codes

Electrocardiograms (EKG/ECG)

The most commonly performed cardiac diagnostic test still needs careful attention to coding:

  • 93000 - Routine EKG with at least 12 leads, including interpretation and report
  • 93005 - EKG tracing only (technical component)
  • 93010 - EKG interpretation and report only (professional component)

When billing globally, use 93000. For hospital-based EKGs where you provide only the interpretation, use 93010 with modifier 26.

Echocardiography

  • 93306 - Transthoracic echo with 2D, M-mode, spectral Doppler, and color flow (complete)
  • 93307 - Transthoracic echo, limited or follow-up study
  • 93308 - Transthoracic echo, limited, without spectral/color Doppler
  • 93350 - Stress echo with exercise or pharmacological stress

Pre-authorization strategies

Cardiac catheterization, stent placement, and advanced imaging typically require pre-authorization. Habits that hold up:

  • Submit authorization requests with clinical notes that support medical necessity
  • Use standardized templates that include patient history, symptoms, and failed conservative treatments
  • Track authorization expiration dates; most expire after 30 to 90 days
  • Appeal denied authorizations with peer-to-peer physician review when available

Global vs. component billing

Knowing when to bill globally versus using modifiers 26 (professional) and TC (technical) is critical for practices that perform tests in both office and hospital settings. Always verify the place of service and who owns the equipment before selecting the billing method.
